Terms and Conditions

Admission: Admission is subject to an assessment to ensure our community can meet the resident’s care needs.
Care Services: Services provided are based on an individualized care plan and may change as the resident’s needs evolve.
Financial Responsibility: Monthly fees are due in advance. Families are responsible for all costs not covered by insurance, Medicaid, or other payment sources.
Spend-Down & Medicaid: Residents beginning as private pay may transition to Medicaid once eligibility is approved, in accordance with state guidelines.
Visitation: Families are encouraged to visit. Visiting hours and safety guidelines are in place for the well-being of all residents.
Personal Belongings: While we take great care to maintain a safe environment, residents and families are responsible for personal items.
Termination of Agreement: Either party may terminate residency with appropriate written notice, as outlined in the resident agreement.

Memory Care/Assisted Living Facility
